Benfotiamine is the fat-soluble form of vitamin B1 (thiamine).Transketolase is an enzyme critical to blood sugar metabolism but for it to be able to work properly it requires the assistance of a cofactor Thiamine BI. Thiamine B1 is though water soluble and hence less available inside of cells. Benfotiamine, on the other hand, being fat-soluble and more bioavailable can more readily penetrate cells and assist the transketolase.

In landmark studies, Benfotiamine effectively assisted and increased transketolase activity in cell cultures by 300% compared to a 20% of thiamine. This activation is needed to stop three out of the four metabolic pathways that leads to damage and destruction of blood vessels.

Benfotiamine also blocks the transcription factor known as nuclear factor Kappa b (NF-kB), which is implicated in a number of numerous inflammatory conditions.
